9. Certificate of Identity comprising photograph issued by a Tehsildar/Gazetted Officer on a letterhead.
10. State/Union Territory Governments/Administrations issued handicapped medical certificate or Disability ID Card.
11. Bhamashah Card.
12. Certificate of Identity comprising photograph issued by an MLA/MLC/MP/Municipal Councilor on a letterhead.
13. Certificate from the head of a recognised educational institution/warden/superintendent/matron.
14. RSBY Card.
15. Gazette notification in case of name change.
16. SC/ST/OBC Certificate comprising photograph.
17. SSLC book comprising photograph.
18. Marriage Certificate comprising photograph.
19. For rural areas – Certificate of Identity comprising photograph issued by the mukhiya or village panchayat head.
